Manage and respond to reports of employee injuries received on the job. The process includes a report to be submitted by the injured employee (or a proxy if the employee is unable), and a subsequent form to ensure appropriate follow up action, including the possible filing of a worker's compensation claim, is taken.

On the job injury report
To be completed by the employee (or a proxy if the employee is unable). You can add site/job specific fields as needed.

Response to injury
To be completed by the HR Officer to ensure that appropriate follow-up and corrective actions are taken in response to an on the job injury. You can modify as needed.
