Military service leave
To be completed by the employee requesting military leave. You can customize the form to include further details such as any organization specific or legal requirements.

FMLA Checklist
To be completed by the Human Resources officer to assess whether the employee qualifies for FMLA and to put them on FMLA if appropriate. You can customize the form to check against other Leave programs or to include other legal or organizational requirements.

Human Resources pay code
To be completed by the HR Officer to ensure that the correct information is entered into the payroll system.
